Financial Information

We have created a comfortable environment where you can feel confident in the level of prosthodontic care you receive. For your first visit, please arrive 15 minutes before your scheduled appointment to complete all insurance and health information forms. Your initial exam will last approximately one hour. This visit will help us get acquainted with each other and learn about your goals and desires for your prosthodontic treatment.

Payment for your prosthodontic consultation is due at the time of your appointment. We accept Visa, MasterCard, Debit Card, Cheque, and Cash. Our office does not take assignment of dental insurance plans. As a courtesy, we are pleased to submit your claim so that you can be reimbursed for any coverage that your plan may provide for dental services. Your carrier pays you directly for your expenses based on your eligible benefits. Insurance coverage is arranged by you and/ or your employer as a benefit. Please direct any questions related to your insurance plan to your employer or directly to the insurance company.

Missed or Cancelled Appointments

Your appointment has been reserved specifically for you. We know that unforeseen emergencies can occur. If you must reschedule your appointment, we ask that a two-business days notice be given so that we can offer your appointment for the care of another patient.
